Diagnosing functional neurological disorder: seeing the whole picture

Functional neurological disorder (FND) is a complex neuropsychiatric syndrome with many phenotypes that are commonly encountered in clinical practice. Despite the heterogeneity of FND, the rate of misidentification is consistently low. For the more common motor subtypes, there are clear positive clinical, electrophysiological, and rarely imaging criteria that can establish the diagnosis in the traditional sense. For nonmotor subtypes, the characterization may be less clear. Here, we argue that the current diagnostic criteria are not reflective of the current shared neuropsychiatric understanding of FND, and, as a result, provide an incomplete picture of the diagnosis. We propose a three-step diagnostic triad for FND, in which the traditional neurological diagnosis is only the first element. Other steps include psychiatric/psychological formulation, integration, and follow-up. We advocate that this diagnostic approach should be the shared responsibility of neurology and mental health professionals. Finally, a research agenda is proposed to address the missing factors in the field.

Background
Psychiatric disorders, including eating disorders (EDs), have clinical outcomes that range widely in severity and chronicity. The ability to predict such outcomes is extremely limited. Machine-learning (ML) approaches that model complexity may optimize the prediction of multifaceted psychiatric behaviors. However, the investigations of many psychiatric concerns have not capitalized on ML to improve prognosis. This study conducted the first comparison of an ML approach (elastic net regularized logistic regression) to traditional regression to longitudinally predict ED outcomes.
MethodsFemales with heterogeneous ED diagnoses completed demographic and psychiatric assessments at baseline (n = 415) and Year 1 (n = 320) and 2 (n = 277) follow-ups. Elastic net and traditional logistic regression models comprising the same baseline variables were compared in ability to longitudinally predict ED diagnosis, binge eating, compensatory behavior, and underweight BMI at Years 1 and 2.
ResultsElastic net models had higher accuracy for all outcomes at Years 1 and 2 [average Area Under the Receiving Operating Characteristics Curve (AUC) = 0.78] compared to logistic regression (average AUC = 0.67). Model performance did not deteriorate when the most important predictor was removed or an alternative ML algorithm (random forests) was applied. Baseline ED (e.g. diagnosis), psychiatric (e.g. hospitalization), and demographic (e.g. ethnicity) characteristics emerged as important predictors in exploratory predictor importance analyses.
ConclusionsML algorithms can enhance the prediction of ED symptoms for 2 years and may identify important risk markers. The superior accuracy of ML for predicting complex outcomes suggests that these approaches may ultimately aid in advancing precision medicine for serious psychiatric disorders.

Background
Many studies have identified changes in the brain associated with obsessive–compulsive disorder (OCD), but few have examined the relationship between genetic determinants of OCD and brain variation.
AimsWe present the first genome-wide investigation of overlapping genetic risk for OCD and genetic influences on subcortical brain structures.
MethodUsing single nucleotide polymorphism effect concordance analysis, we measured genetic overlap between the first genome-wide association study (GWAS) of OCD (1465 participants with OCD, 5557 controls) and recent GWASs of eight subcortical brain volumes (13 171 participants).
ResultsWe found evidence of significant positive concordance between OCD risk variants and variants associated with greater nucleus accumbens and putamen volumes. When conditioning OCD risk variants on brain volume, variants influencing putamen, amygdala and thalamus volumes were associated with risk for OCD.
ConclusionsThese results are consistent with current OCD neurocircuitry models. Further evidence will clarify the relationship between putamen volume and OCD risk, and the roles of the detected variants in this disorder.

Acute and 3-month effects of microcrystalline hydroxyapatite, calcium citrate and calcium carbonate on serum calcium and markers of bone turnover: a randomised controlled trial in postmenopausal women

Ca supplements are used for bone health; however, they have been associated with increased cardiovascular risk, which may relate to their acute effects on serum Ca concentrations. Microcrystalline hydroxyapatite (MCH) could affect serum Ca concentrations less than conventional Ca supplements, but its effects on bone turnover are unclear. In the present study, we compared the acute and 3-month effects of MCH with conventional Ca supplements on concentrations of serum Ca, phosphate, parathyroid hormone and bone turnover markers. We randomised 100 women (mean age 71 years) to 1 g/d of Ca as citrate or carbonate (citrate–carbonate), one of two MCH preparations, or a placebo. Blood was sampled for 8 h after the first dose, and after 3 months of daily supplementation. To determine whether the acute effects changed over time, eight participants assigned to the citrate dose repeated 8 h of blood sampling at 3 months. There were no differences between the citrate and carbonate groups, or between the two MCH groups, so their results were pooled. The citrate–carbonate dose increased ionised and total Ca concentrations for up to 8 h, and this was not diminished after 3 months. MCH increased ionised Ca concentrations less than the citrate–carbonate dose; however, it raised the concentrations of phosphate and the Ca–phosphate product. The citrate–carbonate and MCH doses produced comparable decreases in bone resorption (measured as serum C-telopeptide (CTX)) over 8 h and bone turnover (CTX and procollagen type-I N-terminal propeptide) at 3 months. These findings suggest that Ca preparations, in general, produce repeated sustained increases in serum Ca concentrations after ingestion of each dose and that Ca supplements with smaller effects on serum Ca concentrations may have equivalent efficacy in suppressing bone turnover.

Molluscs from the Middle and Upper Ordovician succession of Girvan, SW Scotland are common and diverse in some localities. The mollusc fauna consists mainly of gastropods, bivalves and various univalved molluscs (mimospirids and tergomyans), along with scarcer polyplacophorans, rostroconchs and cephalopods. The present study gives an overview of the distribution and palaeoecology of bivalves, gastropods and univalved molluscs and compares them with mollusc faunas worldwide. Gastropods, mimospirids and tergomyans are present from the Darriwilian (mid Llanvirn) onwards in both siliciclastic and carbonate facies, and increase in diversity through the Sandbian (Caradoc) and into the Katian (Ashgill). Bivalves first appeared in Girvan in the late Darriwilian (early Caradoc) in deep water siliciclastic facies; where they continued to be more abundant and diverse than in equivalent carbonate facies. Molluscs are initially Laurentian in aspect, though peri-Gondwanan faunal elements occur, particularly during the Sandbian. The pattern of bivalve and gastropod diversity found in the Ordovician of Girvan generally follows that of the known global diversity for these groups.

Sloshing and slamming oscillations in a collapsible channel flow

We consider laminar high-Reynolds-number flow through a finite-length planar channel, where a portion of one wall is replaced by a thin massless elastic membrane that is held under longitudinal tension T and subject to a linear external pressure distribution. The flow is driven by a fixed pressure drop along the full length of the channel. We investigate the global stability of two-dimensional Poiseuille flow using a method of matched local eigenfunction expansions, which is compared to direct numerical simulations. We trace the neutral stability curve of the primary oscillatory instability of the system, illustrating a transition from high-frequency ‘sloshing’ oscillations at high T to vigorous ‘slamming’ motion at low T. Small-amplitude sloshing at high T can be captured using a low-order eigenmode truncation involving four surface-based modes in the compliant segment of the channel coupled to Womersley flow in the rigid segments. At lower tensions, we show that hydrodynamic modes increasingly contribute to the global instability, and we demonstrate a change in the mechanism of energy transfer from the mean flow, with viscous effects being destabilizing. Simulations of finite-amplitude oscillations at low T reveal a generic slamming motion, in which the flexible membrane is drawn close to the opposite rigid wall before recovering rapidly. A simple model is used to demonstrate how fluid inertia in the downstream rigid channel segment, coupled to membrane curvature downstream of the moving constriction, together control slamming dynamics.

Background
Clinical samples have identified a number of psychosocial risk factors for suicidal acts but it is unclear if these findings relate to the general population.
AimsTo describe the prevalence of and psychosocial risk factors for suicidal acts in a general adult population.
MethodData were obtained from a Canadian epidemiological survey of 36 984 respondents aged 15 years and older (weighted sample n=23 662 430).
ResultsOf these respondents, 0.6% (weighted n=130 143) endorsed a 12-month suicidal act. Female gender (OR=4.27, 95% CI 4.05–4.50), being separated (OR=37.88, 95% CI 33.92–42.31) or divorced (OR=7.79, 95% CI 7.22–8.41), being unemployed (OR=1.70, 95% CI 1.50–1.80), experiencing a chronic physical health condition (OR=1.70, 95% CI 1.67–1.86) and experiencing a major depressive episode in the same 12-month period as the act (OR=9.10, 95% CI 8.65–9.59) were significantly associated with a suicidal act.
ConclusionsThe psychosocial correlates of suicidal acts in this sample are consistent with those previously reported in clinical and general population samples. These findings reinforce the importance of the determination of suicide risk and its prevention not only of psychiatric illness but of physical and psychosocial factors as well.

Locally abundant and diverse brachiopod faunas, associated with unstable outer shelf and slope environments, occur throughout the Barr and Ardmillan groups (middle Llanvirn–upper Ashgill) in the Girvan district of SW Scotland. A dataset of 350 brachiopod species from 30 horizons through the Middle–Upper Ordovician succession forms the basis for a description of brachiopod diversity through the succession and comparison with global patterns and trends through this time interval. The Middle Ordovician Barr Group incorporates shallow water carbonate and clastic facies, characterised by Valcourea confinis, with deeper-water facies. The trilobite-dominated Albany Group preserves outer shelf biofacies. Deep-water facies occur in the Balclatchie Formation of the lower Ardmillan Group, including an early occurrence of a Foliomena-type species. High diversity brachiopod faunas occur in clastic facies, though many of the biofacies have a transported component to them. Representatives of the deep-water Foliomena fauna occur intermittently throughout the Upper Ardmillan Group, appearing in both the Whitehouse and Drummuck subgroups. This distinctive assemblage of small, thin-shelled brachiopods, including Dedzetina, Christiania, Cyclospira and Foliomena itself, is interbedded with a variety of other less cosmopolitan deep-water assemblages, including the Onniella–Skenidioides and Lingulella–Trimurellina associations. Shallower-water environments in the middle Ashgill Lower Drummuck Subgroup hosted the Fardenia–Eopholidostrophia association in sands, and the Christiania–Leptaena association in muds and silts. The remarkable Lady Burn Starfish Beds in the upper part of the group contain a variety of brachiopod-dominated assemblages, including the Eochonetes and Plaesiomys–Schizophorella associations, transported from various shelf locations, within a very diverse mid Ashgill biota. The upper Ashgill High Mains Formation contains abundant elements of the terminal Ordovician Hirnantia fauna, including Eostropheodonta, Hindella and Hirnantia itself, but also some taxa more typical of the Laurentian Edgewood Province. As a whole, the changing brachiopod biodiversity biofacies reflect environmental fluctuations, on this part of the Laurentian margin, driven by mainly eustatic and tectonic events against a background of global biotic radiation.
